Otherwise the numbering of steps gets broken. Removing the empty lines
between <div> and <p> contradicts what we've been doing so far but
I couldn't otherwise find a way of having notes inside a step without
having ikiwiki adding ugly extra vertical space.

I'm sorry the diff of this commit is hard to read but I don't think I
can do better, that's Git's diff algo.
